  1. The unrealistic thing about this scene is that the doors to the dogs have to remain open while Ramsay is unconscious. Sansa has to stand around for hours until night fall, waiting for Ramsay to regain consciousness. The dogs then have to hungrily wait until Ramsay and Sansa have their exchange before this happens.
There could be logical continuity. Lets say Sansa has Jon's direwolf unleashed on Ramsay.
But the point is fantasy. This is a revenge fantasy in which Sansa is shown to have full composure, being totally cool and unmoved, even during a battle, and is doing something yet not doing it.
For example, when the Hound wants to kill those guys for killing the house building community, the Brotherhood without Banners insists on no cruel and unusual punishment. Jon refused to do that to those who murdered him. But here Sansa is getting her revenge, while keeping her hands supposedly clean because Ramsay brought it on himself by not feeding the dogs?
Anyway. The actor who played Ramsay was awesome throughout most of the series.

  2. I think "They were. Now they're starving" is a metaphor for if Ramsey had continued to rule the North. The houses would initially obey but then wouldn't receive the support they needed and would betray them if this battle hadn't happened. Also, how the hell did they get Ramsey in there, tie him up, unlock the kennel doors and get out before the hounds got them?
