Based on the showrunners, young Hodor witnessed and experienced his own death, while older future Hodor was dying. That is pretty much at the point young Hodor starts saying, "Hodor" forever; future Hodor is dead and the mind is blurred as are the words "hold the door". Very sad. All this time, his destiny was to hold the door and save Bran.
